Langues : Anglais (eng) Mots-clés : ENSEIGNEMENT PSYCHIATRIE REPRESENTATION SISMOTHERAPIE ETUDIANT EN SOINS INFIRMIERS MEDECIN Résumé : Background : This study explored the impact of theoretical and practical teaching on electro convulsive therapy (ECT) on medical and nursing students’ stigmatizing attitudes towards ECT and representations of it. Method : Fourth-year medical students and nursing students answered questions from the Questionnaire on Attitudes and Knowledge of ECT (QuAKE) and from the Mental Illness: Clinicians’ Attitudes version 2 (MICA v2) scale. The questionnaires were completed before and after observing a 3-hour practical training session in the ECT unit. The endpoint was the impact of practical training as assessed by MICA and QuAKE scores. Multivariate analyses were used to explore the impact of practical training on MICA and QuAKE total scores. Results : Stigmatizing attitudes and representations of both medical and nursing students towards ECT were reduced after practical training (β=–4.43 [95% CI –6.15; –2.70] p=0.0001). The impact was greater in medical students (β=–8.03; 95% CI [–10.71; –5.43], P =0.0001) than in nursing students (β=–2.77; 95% CI [–4.98; to 0.44], P =0.02). Gender, psychiatric history in close persons, and having already followed a psychiatric/ECT course had no independent impact on stigmatizing attitudes towards ECT and representations of it. Conclusion : Practical training in ECT should be given to all health professionals to improve access to it. Langues : Anglais (eng) Mots-clés : DSM 5 NEUROPSYCHIATRIE DIAGNOSTIC DEMENCE DE TYPE ALZHEIMER DEPRESSION MAJEURE FONCTION COGNITIVE FACTEUR DE RISQUE PREVALENCE Résumé : Objective : The diagnosis of Major Depressive Disorder (MDD) is based on the DSM-V criteria and is established by a clinician. This allows quantifying depression based on clinical criteria. As such, MDD differs from other types of depressions that are measured by subjective scales. Here, we evaluated the MDD risk factor on other neuropsychiatric symptoms (NPS) as well as MDD association with cognitive performance in Alzheimer's disease (AD), Mild Cognitive Impairment (MCI) and Healthy Controls (HC). Methods : Data of 208 patients with AD, 291 patients with MCI and 647 HC were extracted from the National Alzheimer's Coordinating Center database. All participants included in this study were assessed by a physician for the MDD criteria, underwent an NPS evaluation using the NeuroPsychiatric Inventory, and a comprehensive cognitive assessment. Participants were classified as being with and without MDD. We performed logistic regression and MANCOVA models respectively with NPS and cognitive performance as variables of interest and MDD as fixed factors within each group. The MANCOVA was controlled for the effects of age, sex, and education. Results : MDD increased the risk for psychotic, affective and behavioral NPS in MCI, as well as affective and behavioral NPS in HC and AD. Also, MCI with MDD had lower performance on selective attention and mental flexibility. Conclusions : MDD seems to increase the probability of a prevalence of NPS in all groups (HC, MCI and AD). Longitudinal data processing would help to understand the neuropsychiatric evolution of elderly subjects with MDD. Langues : Anglais (eng) Mots-clés : DEPRESSION RESISTANTE TROUBLE BIPOLAIRE DOPAMINE INTERACTION MEDICAMENTEUSE TOLERANCE MEDICAMENTEUSE MEDICAMENT INHIBITEUR SPECIFIQUE DE LA RECAPTURE DE LA SEROTONINE Résumé : Introduction : Dopamine (DA) is likely to be involved in some depressive dimensions, such as anhedonia and amotivation, which account for a part of treatment-resistant forms. Monoamine oxidase inhibitors (MAOI) and direct D2 and D3 receptors agonists (D2/3r-dAG) are known to help, but we lack safety data about their combined usage. We report on safety and tolerance of the MAOI+D2r-dAG combination in a clinical series. Method : All patients referred to our recourse center for depression between 2013 and 2021 were screened to select those who did receive the combo. Data were extracted from clinical files. Results : Sixteen patients of 60±17 years of age (8 women, 7 with age>65years, all suffered from treatment resistant depression, 7 with bipolar disorder) received the combo. There were no life-threatening adverse effects (AE). However, AE were reported by 14 patients (88%) most of which were mild and consisted of insomnia, nausea, nervousness, confusion, impulse control disorder and/or 'sleep attacks'. One patient presented a serious AE requiring a short hospitalization for confusion. Intolerance led to failure to introduce treatment in two patients (13%). The retrospective non-interventional design, the variety of molecules, and the modest sample size limited the scope of these results. Conclusion : There was no life-threatening safety issue in combining MAOI and D2/3r-dAG, especially regarding cardiovascular side effects. The systematic screening of AE might account for their frequency, but these precluded the treatment in only two patients. Comparative studies are needed to assess the efficacy of this new combination. This study sought to assess the psychological impact (stress, burnout and self-efficacy) of the COVID-19 outbreak in French GPs. Methods : We carried out a postal-based survey of all GPs who worked in the French region of Normandy (departments of Calvados, Manche and Orne) from the exhaustive database of the Union Régionale des Médecins libéraux (URML Normandie) as of 15th April 2020 (one month after the first French COVID-19 sanitary lockdown). The second survey was conducted four months later. Four validated self-report questionnaires were used at both inclusion and follow-up: Perceived Stress scale (PSS), Impact of Event Scale-revised (IES-R), Maslach Burnout Inventory (MBI) and General Self-Efficacy scale (GSE). Demographic data were also collected. Results : The sample consists of 351 GPs. At the follow-up, 182 answered the questionnaires (response rate: 51.8%). The mean scores of MBI significantly increased during follow-up [Emotional exhaustion (EE) and Personal accomplishment, P < 0.01]. Higher burnout symptoms were found at the 4-month follow-up in 64 (35.7%) and 86 (48.0%) participants (43 and 70 participant at baseline), according respectively to EE and depersonalisation scores (P = 0.01 and 0.09, respectively). Conclusion : This is the first longitudinal study that has shown the psychological impact of COVID-19 in French GPs. Based on validated a self-report questionnaire, burnout symptoms increased during follow-up. It is necessary to continue monitoring psychological difficulties of healthcare workers especially during consecutive waves of COVID-19 outbreak. The aim of this study was to estimate the prevalence of psychiatric disorders using standardised and validated research instruments and examine the effect of exposure to trauma. Methods : One hundred and ten youth (92.7% male, median age 19.7 [18.1–22.8]) from Chambery, Montpellier and La Rochelle were recruited to a cross-sectional exploratory study. During a face-to-face interview, somatoform disorder, anxiety, and depression were assessed using the Patient Health Questionnaire (score ≥ 10) and post-traumatic stress disorder (PTSD) with the PTSD Checklist for DSM-5 (score ≥ 33). Traumatic life events were assessed using the Life Events Checklist. Results : Of the youth, 19.3% had a probable somatoform disorder, 17.6% anxiety, 28.7% depression, and 20% PTSD. The number of traumatic life events increased the risk of depression (multi-adjusted OR (95%CI): 1.56 (1.25–1.96)), PTSD (1.60 (1.23–2.08)), somatoform disorder (1.41 (1.10–1.82), and anxiety (1.33 (1.02–1.72)). Physical assault was the type of event positively associated with the most disorders (P ≤ 0.01, except for anxiety), followed by witnessing sudden and violent death (P ≤ 0.01 for depression and PTSD) and sexual assault (P = 0.002 for PTSD). Conclusion : Our study highlights the high prevalence of psychiatric disorders in young adults who arrived as UASC and the impact on their mental health of cumulative trauma and exposure to interpersonal and violent traumatic life events. A greater focus on their mental health with regular assessments is needed in order to provide rapid and adapted care. From a psychological point of view, it is possible to consider transdiagnostic processes that would participate in both the dysregulation of pain and associated emotions. Objective : The aim of this study was to test the links that exist between the tendency to Repetitive Negative Thinking (RNT) and the anxious-depressive manifestations in fibromyalgia. More specifically, we wanted to test a double mediation model where RNT would mediate the link between pain and depression/anxiety via catastrophizing. Method : Eighty-two patients with fibromyalgia completed a series of questionnaires evaluating their level of depression, anxiety, disability related to pain, catastrophizing as well as various measures of Repetitive Thoughts. Results : The results showed strong correlations between RNT levels, pain, and anxious-depressive manifestations in this population. Moreover, the links between pain and depression/anxiety were mediated by catastrophizing and RNT in serial. Conclusion : Results support the interest of studying RNT as a transdiagnostic process in fibromyalgia pain. Considering RNT in fibromyalgia allows a better understanding of tthe links that exist between pain and emotional disorders in this population and thus to better understand the psychopathological comorbidity of fibromyalgia.[Résumé d'auteur]

Langues : Anglais (eng) Mots-clés : PARCOURS DE SOINS USAGE COMPLIANCE URGENCE PSYCHIATRIQUE FACTEUR DE RISQUE DEPISTAGE JEUNE ADULTE Résumé : Introduction : Young individuals constitute a key population for the screening of problematic use of substances (PUS), but they are not likely to seek support and are hard to reach. Targeted screening programs should thus be developed in the places of care they may attend for other reasons, including emergency departments (EDs). We aimed to explore the factors associated with PUS in young people attending an ED; we measured the subsequent access to addiction care after ED screening. Methods : This was a prospective interventional single-arm study which included any individual aged between 16 and 25 years who attended the main ED of Lyon, France. Baseline data were sociodemographic characteristics, PUS status using self-report questionnaires and biological measures, level of psychological health, and history of physical/sexual abuse. Quick medical feedback was provided to the individuals presenting a PUS; they were advised to consult an addiction unit, and contacted by phone at three months to ask whether they had sought treatment. Baseline data were used to compare PUS and non-PUS groups using multivariable logistic regressions, to provide adjusted odds ratios (aORs) and 95% confidence intervals (95% CI), with age, sex, employment status, and family environment as the adjustment variables. The characteristics of PUS subjects who subsequently sought treatment were also assessed using bivariable analyses. Results : In total, 460 participants were included; 320 of whom (69.6%) were presenting current substance use, and 221 (48.0%) with PUS. Compared to non-PUS individuals, PUS ones were more likely to be males (aOR = 2.06; 95% CI [1.39–3.07], P < 0.001), to be older (per one-year increase: aOR = 1.09; 95% CI [1.01–1.17], P < 0.05), to have an impaired mental health status (aOR = 0.87; 95% CI [0.81–0.94], P < 0.001), and to have a history of sexual abuse (aOR = 3.33; 95% CI [2.03–5.47], P < 0.0001). Only 132 (59.7%) subjects with PUS could be reached by phone at 3 months, among whom only 15 (11.4%) reported having sought treatment. Factors associated with treatment seeking were social isolation (46.7% vs. 19.7%; P = 0.019), previous consultation for psychological disorders (93.3% vs. 68.4%; P = 0.044), lower mental health score (2.8 ± 1.6 vs. 5.1 ± 2.6; P < 0.001), and post-ED hospitalization in a psychiatric unit (73.3% vs. 19.7%; P < 0.0001). Discussion/Conclusion : EDs are relevant places to screen PUS in youth, but the level of seeking further treatment needs to be substantially improved. Offering systematic screening during an emergency room visit could allow for more appropriate identification and management of youth with PUS. In order to explore the control of innate immune responses in SZ, we aimed to investigate the potential association between twelve TLR2, TLR4 and TLR9 variants (TLR2: rs4696480T > A, rs3804099T > C, rs3804100T > C; TLR4: rs1927914G > A, rs10759932T > C, rs4986790A > G, rs4986791T > C, rs11536889G > C, rs11536891T > C; TLR9: rs187084A > G, rs352139T > C and rs352140C > T) and SZ susceptibility in a Tunisian population. Patients and methods : This study included 150 patients and 201 healthy controls with no history of psychiatric illness. Genotyping was done using a TaqMan SNP genotyping assay. We also assessed a haplotype analysis for TLR2, TLR4 and TLR9 variants with SZ using Haploview 4.2 Software. Results : We found that the AA genotype of the TLR2 rs4696480T > A variant was significantly associated with an increased risk of SZ (46% vs. 31%, P = 4.7 × 10−3, OR = 1.87 and 95% CI [1.18–2.97]). The frequency of the TA genotype was significantly higher in the control group than in SZ patients (27% vs. 43%, P = 2.1 × 10−3) and may be associated with protection against SZ (OR = 0.49 and 95% CI [0.30–0.80]). Whereas, the TLR9 rs187084-GG genotype was higher in the control group compared to patients (16% vs. 5%, P = 1.6 × 10−3) and would present protection against SZ (OR = 0.28, CI = [0.10–0.68]). The ACT haplotype of the TLR2 and the ACC haplotype of the TLR9 gene were identified as a risk haplotypes for SZ (P = 0.04, OR = 9.30, 95% CI = [1.11–77.71]; P = 3 × 10−4, OR = 6.05, 95% CI = [2.29–15.98], respectively). Conclusion : The results indicate that TLR2 and TLR9 genetic diversity may play a role in genetic vulnerability to SZ. However, including more patients and evaluation of TLR2 and TLR9 expression are recommended.
Ils consommaient également davantage de psychotropes (OR=1,38 ; IC95 [1,05–1,81]). Langues : Anglais (eng) Mots-clés : TROUBLE DU SPECTRE DE L'AUTISME SYNDROME D'ASPERGER HUMOUR IRONIE ECHELLE D'EVALUATION REVUE DE LA LITTERATURE Résumé : Objectives : Humor is essential to social relationships. Its use and understanding appear to be impaired in people with Autism Spectrum Disorder (ASD). The main objective was to review the existing literature on the detection, understanding and use of humor in persons with ASD. The secondary objective involved exploring assessment scales and specific intervention tools. Methods : A systematic review of the literature was carried out on all available French and English scientific papers about humor – including irony – in persons with ASD up to November 2021. We extracted 552 references and included 43 articles from six databases. Results : Studies suggest that those with ASD can detect audiovisual and written humor. Understanding humor was impaired in writing and when using pure auditory stimuli and non-verbal cartoons. For irony, the results indicated a lower detection of quality and less understanding in speaking but not in writing. Regarding its use, in terms of expression, people with ASD use benevolent humor less often and do not consider humor as a key source of satisfaction with life, as opposed to the control group. Conclusions : It appears that it would be worthwhile to develop standardized humor detection and assessment tools specific to persons with ASD. Practical strategies that focus on humor ability could be worth developing, either individually or in groups. All operating 'cures more wonderful than each other'. This book questions in a very relevant way the role of the power of the spirit, of healing by the spirit and therefore of suggestion. This 'art' of healers includes an affective and emotional coefficient which constitutes its essential spring. There is a mysterious force between the sick and the physician that goes 'soul to soul'. Current medicine is still very much borrowed from its past with a remnant of 'magical' thought. Also, the empathetic dimension of the care relationship remains one of the most necessary dimensions for the exercise of the medical profession. Langues : Anglais (eng) Mots-clés : COVID-19 VACCINATION CROYANCE THEORIE DU COMPLOT Résumé : Recent years saw a drastic surge in conspiracy theories, ranging from climate-change denial to suspicion about the real motives behind COVID-19 control measures. A better understanding of the cognitive roots of these unfounded and rigid beliefs is of crucial importance. We need to learn from this pandemic, overcome the population's rejection of science and support critical thinking at a societal level. Notably, conspiracy theories have been proposed not only as false beliefs, but also as a way of making sense of events occurring in a context of great social uncertainty. Interestingly, previous work underscored that perceiving some irrelevant elements of our environment as abnormally important, a phenomenon called aberrant salience attribution, was associated with the endorsement of rigid beliefs, such as delusional ideations, or the acceptance of scientifically doubtful facts. Surprisingly, despite some similarities in the unshakable nature of these belief categories, the presence of aberrant salience among people embracing conspiracy theories has never been properly explored.
