Langues : Anglais (eng) Mots-clés : RELATION PARENT ENFANT CANADA PROGRAMME D'INTERVENTION EVALUATION TROUBLE DU COMPORTEMENT STRESS ENFANT Résumé : Objectives : Behavior problems are one of the most common reasons for seeking mental health services in pediatric populations. The objectives are to evaluate the effects of the EQUIPE program (Étude Québécoise d’Intervention pour les Parents d’Enfants avec des problèmes de comportement) and to analyze the impact of the severity of behavior problems and of parental characteristics. Methods : This program was translated from the Community Parent Education Program. The effects of EQUIPE, as compared to a control group, were evaluated by using Child Behavior Checklist and Parent Stress Index questionnaires before (T0) and after the intervention program (T1), and at 6 (T2) and 12 months (T3) follow-up visits. Results : In total, 533 participants were enrolled in intervention (n = 465) (with “severe” or “mild” subgroups according to CBCL-T score) and a control group (n = 68). In the two groups, the results showed a statistically significant decrease in Child Behavior Checklist and Parent Stress Index total scores at T1, T2 and T3, with the exception of Child Behavior Checklist total scores in the control group at T2. In the intervention group Child Behavior Checklist total scores were significantly higher in the “severe”; which was not the case for Parent Stress Index total scores at T2 and T3. Discussion : Socioeconomic characteristics, family details, parental medical history and the age of the children appeared to influence changes in Child Behavior Checklist and Parent Stress Index total scores. Conclusion : The EQUIPE program is an effective intervention for reducing behavior problems and parents’ stress in a French-Canadian population. Langues : Anglais (eng) Mots-clés : ETUDIANT COVID-19 CONSEQUENCE STRESS CONFINEMENT PANDEMIE SANTE MENTALE PROFESSIONNEL DE SANTE Résumé : In France, care workers and health students have been intensely mobilized during the first wave of the COVID-19 pandemic. But few studies have evaluated psychological distress on non-medical health students, in addition to the challenges posed by pedagogical continuity while universities are closed following health and safety regulations. Objectives : This study aims to assess COVID-19's impact on health students in France on different levels: psychological, educational and social. Methods : An online national cross-sectional study, from April 11 to May 30 2020, included sociodemographic, work conditions and numeric scales. Results : A total of 4411 students answered. Regarding the K6 scale, 39% of students had moderate distress, and 21% had a high level of distress. Risk factors of psychological distress included being a woman (P < 0.001), being between 19 and 21 years old (P < 0.001), living alone (P = 0.008), and not having the ability to isolate (P < 0.001). Students on the frontline had less psychological distress (57 vs 62%, P = 0.003), better quality of sleep (34% vs 28% high quality, P < 0.001) but a higher consumption of medical (8.5% vs 6.5%, P = 0.044) and non-medical (18% vs 10%, P < 0.001) psychotropic drugs. Nurse and medical students had more distress and used more non-medical psychotropic substances than other health students (15% vs 9.2%). Discussion : COVID-19’ crisis had an important impact on health students’ mental health, social life and training with discrepancies regarding the speciality whether they were on the frontline or not. There is an urgent need for psychological and pedagogical support for students, and even more so regarding the prolongation of the COVID-19 epidemic. However, only a few studies have focused on the interaction among these processes. Preliminary studies have identified a moderation effect of rumination on the link between thought content and emotional difficulties, and a mediation effect of ruminations on the link between mindfulness and emotional difficulties. Method : We recruited 236 participants (185 women) who consented online to participate by choosing to either continue with the study or decline to proceed. They completed a battery of questionnaires online, namely Positivity scale, General Health Questionnaire, Rumination Response Scale, Five Facet Mindfulness Questionnaire and Cognitive Fusion Questionnaire. Results : All cognitive processes were significantly correlated with emotional distress. Step-by-step linear regression analysis revealed that positivity, cognitive fusion and brooding were significant independent predictors of emotional difficulties. Bootstrapping analysis confirmed that cognitive fusion and brooding mediate the link between mindfulness and depression and anxiety-insomnia. They also demonstrated that cognitive fusion moderates the link between positivity and depression but not anxiety-insomnia. Conclusion : Cognitive processes interact with each other. Taken together, these results suggest that combining cognitive interventions is not useful and that different cognitive interventions may be selected depending on the patient's profile. The purpose of the present study was to examine the reliability and validity of the Arabic version of the Beck Cognitive Insight Scale. Methods : The English language version of the Beck Cognitive Insight Scale was translated into Literary Arabic. A total of 150 patients with and without psychosis completed the Arabic version of Beck Cognitive Insight Scale and additional evaluations to assess psychopathology. Psychometric properties including content validity, construct validity using confirmatory factor analysis, discriminant validity and reliability were assessed for this translated measure. Results : Our results revealed that the Arabic version of the Beck Cognitive Insight Scale showed adequate psychometric properties that permitone to assess impairments of cognitive insight in patients with psychosis and in patients without psychotic features. The fit indices of the confirmatory factor analysis supported the validity of the two-factor structure corresponding to self-reflectiveness and self-certainty subscales. Psychotic patients were significantly more confident in their beliefs compared with non-psychotic patients. Scores of self-certainty were significantly higher in psychotic patients. Cronbach's alpha reliability indicator of the Arabic version was equal to 0.60. Cronbach's alpha coefficients were 0.60 and 0.53 for the self-reflectiveness sub-scale and the self-certainty sub-scale, respectively. Conclusions : The Arabic version of the Beck Cognitive Insight Scale is valid and reliable for the assessment of cognitive insight. It may improve the detection and the prevention of impairments in cognitive insight. The aim of the current study is to have a better understanding of links between specific body checking behaviors and muscle dysmorphia in social network context. Methods : In total, 342 students practicing weightlifting at the university gym in Bordeaux answered to a survey with sociodemographic information and body checking symptoms including taking specific selfies of muscles and muscularity “Muscle Pics” and the MDDI (Muscle Dysmorphic Disorder Inventory). Results : Muscle dysmorphia was prevalent in 18.7% of our population (64 students). We observed that muscle dysmorphia was correlated to 'Muscle Pics', 'Follow-up', 'Message', 'Selfie', and gym mirror checking with significant results (P < 0.01). Also, 'Muscle Pics' were linked to APEDs use, pro-muscularity websites, fitness model comparison and gym mirror checking (P < 0.01). For muscle dysmorphia, 'Muscle Pics' have strong predictive results (OR = 5.10, P = 0.000) and (OR = 4.08, P = 0.000) for adjusted. 'Follow up' (OR = 4.76, P = 0.000) and (OR = 3.83, P = 0.000) for adjusted, 'Muscle Pics Selfie' (OR = 11.20, P = 0.000) and (OR = 11.55, P = 0.000) for adjusted, 'Muscle Pics Message' (OR = 4.49, P = 0.001) and (OR = 5.78, P = 0.001) for adjusted. Conclusion : 'Muscle Pics' showed several links with muscle dysmorphia for global score 'drive for size', 'functional impairment' but not for 'appearance intolerance' dimension. Pro-muscularity websites, fitness model comparisons and gym mirror checking are linked to muscle dysmorphia and 'Muscle Pics'. Future research on 'Muscle Pics' will help to provide a better understanding of muscle dysmorphia and its link with pro-muscularity influence websites. A cross-sectional comparative study was conducted using the Ask Suicide Screening Questions (ASQ) as a screening tool that targeted all patients presenting for a psychiatric emergency in a university hospital in Beirut during a four-month period. One hundred and three patients of all age groups have been divided into a group of patients with a positive suicidal screening (n = 67, 65%), and another one with a negative suicidal screening (n = 36, 35%). Suffering from a personality disorder was found to be a positive correlate of suicide screening (OR: 21.6, 95% CI: 2.6–179.0). Female gender (OR: 4.5, 95% CI: 1.6–13.2) and an elevated number of previous hospitalizations were found to be positive correlates of past suicidal attempts. These correlates should be assessed in the emergency room department (ER) to prevent any subsequent suicidal behavior. Moreover, data are lacking on outcome predictive factors. This article presents a short synthesis of previous research about outcome predictive factors in child psychiatry. It also describes the protocol of a longitudinal observational European multicentre study the main objective of which was to identify predictive factors of behavioural and emotional disorder outcome in toddlers after parent-child psychotherapy. The secondary objectives were to study predictive factors of the outcome in parents (anxiety-depression symptoms) and parent-child relationship. Method : In order to highlight medium-effect size, 255 toddlers (age: 18 to 48 months) needed to be included. Outcomes will be assessed by comparing the pre- and post-therapy scores of a battery of questionnaires that assess the child's symptoms, the parents’ anxiety/depression, and the parent-child relationship. Multivariate linear regression analysis will be used to identify predictive factors of the outcome among the studied variables (child age and sex, socio-economic status, life events, disorder type, intensity and duration, social support, parents’ psychopathology, parents’ attachment, parent-child relationships, therapy length and frequency, father's involvement in the therapy, and therapeutic alliance). Expected results and conclusion : This study should allow identifying some of the factors that contribute to the outcome of externalizing and internalizing disorders, and distinguishing between pre-existing and treatment-related variables. It should also help to identify children at higher risk of poor outcome who require special vigilance on the part of the therapist. It should confirm the importance of therapeutic alliance. Langues : Anglais (eng) Mots-clés : ADDICTION ALCOOL TABAC RESEAUX SOCIAUX DEPRESSION SUGGESTIBILITE ANXIETE Résumé : Several health issues related to the us of social networking sites (SNS) are documented. One concept that is not clearly studied is suggestibility as the tendency of a person to accept and internalize communication. The aim of the study is to test a model in which suggestibility of people can predict SNS use, which in turn can predict substance use. Methods : Participants answered a questionnaire measuring suggestibility, alcohol consumption, dependence to nicotine and Facebook use. Linear regression and a structural equation model (SEM) were carried out to assess which utilization of Facebook variables explained best the level of alcohol consumption or nicotine dependence and to assess how suggestibility predicted Facebook use. Results : Linear regression analyses indicate that only the number of friends on Facebook is a predictor of the level of alcohol consumption (P < 0.001). Suggestibility predict the frequency of Facebook use (P < 0.001) and of the number of friends on Facebook (P = 0.022). The SEM demonstrates that participants with a higher level of suggestibility have more friends on the SNS, which can influence substance consumption. Conclusion : Individuals’ level of suggestibility affects Facebook use. People with a higher level of suggestibility are more likely to have many friends on the SNS resulting in a possible environmental influence on substance use. The risks engendered by this treatment requires that their prescription and administration be made safer. The frequency of administration of pro re nata depends mainly on the nurse's clinical judgment. Aims : Our first objective was to assess nurses’ satisfaction about the quality of doctors’ pro re nata prescriptions. Our second objective was to assess the nurses’ self-reported practices for administering pro re nata treatments as written in the prescription. Method : Self-administered questionnaires were sent by the hospital's internal mail between November 13, 2014, and December 10, 2014 to all nurses in our psychiatric establishment in France. The questionnaire included multiple-choice questions and questions based on clinical vignettes. Results : The response rate was 51.9% (124 out of 239). Overall, 75.6% considered that the quality of the prescriptions in terms of dosage was satisfactory. However, regardless of the quality of the doctor's pro re nata prescription, nurses did not contact the doctor even when the prescription quality was poor. Unexpectedly, we found that 88.7% have administered medication 'as needed' without a doctor's prescription and sometimes acted without consulting doctors. Conclusions : The nurses appeared globally satisfied with doctors’ prescriptions of pro re nata medications. On the other hand, most administered some medications without any prescription, that is, illegally. Physicians must be rigorous in the quality of their PRN prescriptions. At the same time, nurses must comply with the medical prescription or contact the physician if the quality of the PRN prescription appears poor. They completed the Mental Illness Clinicians Attitudes scale (MICA) and a self-questionnaire exploring sociodemographic data, the university course, social perception and knowledge towards mental illness, and the potential consideration of psychiatry as a career. Results : The average age of participants was 21.73 years (SD ± 2.60), and 72.4% (n = 304) of respondents were female (M F gender ratio = 0.39). The participants held stigmatizing attitudes and negative social perceptions towards mental illness, as indicated by the high score of the MICA scale (mean = 57.24; SD ± 9.95). Females (p = 0.02) and married students (p = 0.02) showed significantly more tolerant attitudes. The attitudes of students (p = 0.37) who completed the psychiatry clerkship were slightly more favorable (p = 0.15). There was no significant difference in attitudes according to the level of study (p = 0.06). Students with a lower socioeconomic level tended to be less stigmatizing (p = 0.08). The assessment of knowledge about mental illness among students objectified major gaps. A total of 17.9% (n = 75) considered psychiatry as a career. Conclusion : The attitudes of medical students in this study were stigmatizing, and this should without delay motivate Moroccan research, educational and health authorities to investigate further scientific research in this area to address these attitudes and remedies. Langues : Anglais (eng) Mots-clés : CRACK USAGE MARTINIQUE TROUBLE DEFICITAIRE DE L'ATTENTION HYPERACTIVITE TROUBLE DE L'HUMEUR IMPULSIVITE PREVALENCE Résumé : Crack consumption is a major public health issue in Martinique with a poor prognosis. A preliminary study has found a high prevalence of history of childhood ADHD (C-ADHD) in crack users. Objective : To determine the prevalence of C-ADHD and adult ADHD (A-ADHD) in crack users and their potential associations with substance use behavior. Methods : All consecutive patients consulting in the public academic hospital covering 376,000 inhabitants were included in the present study and received a comprehensive battery measuring addictive behavior, psychiatric and somatic comorbidities. C-ADHD groups and A-ADHD groups were defined with the Wender-Utah Rating Scale-25 and the Brown ADD Rating Scale, respectively. Impulsivity was evaluated with the Barratt Impulsiveness Scale (BIS-11). Findings : In total, 111 participants were evaluated. Among them, 50 (45%) were classified in the C-ADHD group and 20 (18%) in the A-ADHD group. Compared to the patients without ADHD, those with ADHD were found to have higher impulsivity (C-ADHD: BIS total score 67.90 (10.1) vs. 63.28 (10.5), P = 0.021, BIS attentional score 17.5 (3.6) vs. 15.3 (3.4), P = 0.002, A-ADHD: BIS total score 75.1 (11.3) vs. 63.4 (9.2), P < 0.001, BIS motor impulsivity 26.9 (5.3) vs. 22.6 (4.3), P < 0.001, BIS attentional score 19.3 (3.3) vs. 15.6 (3.5), P < 0.001, BIS planification 28.9 (5.7) vs. 25.10 (4.7), P = 0.003). Fifty percent of A-ADHD patients were found with high impulsivity vs. 15% of patients without A-ADHD (P < 0.001). However, ADHD was not associated with more severe addictive behavior or history of legal consequences. Interpretation : ADHD prevalence is high in cocaine-crack users and associated with increased impulsivity. However, neither ADHD nor impulsivity explains addictive behaviors or legal consequences. It succeeds the Declaration of Hawaii of 1977 and the Declaration of Madrid of 1996. The Code is divided into four sections that cover the practice of psychiatry in clinical practice, education, research and publication as well as public mental health. We will discuss the new ethical issues that this Code raises in parallel with the development of psychiatry and psychiatrists’ role in society in recent years.
