Langues : Anglais (eng) Mots-clés : ADDICTION COMPORTEMENTALE TUNISIE FACTEUR DE RISQUE PREVALENCE TRAVAIL ENQUETE Résumé : Introduction : Workaholism is an emerging form of behavioural addiction encountered in the workplace. The present study aims to assess the prevalence and the associated factors of this phenomenon in engineers. Methods : A cross sectional survey was conducted for two months by means of an online questionnaire of engineers practising in Tunisia. The evaluation of workaholism was based on the WART questionnaire (Work Addiction Risk Test). Results : A total of 107 engineers have answered the questionnaire. The mean age of participants was 29.2 ± 4.4 years. Computer engineers represented 32.7% of our sample. Most of engineers worked more than 8 hours per day (45.8%) and less than 6 days per week (63.6%). A high risk of workaholism was noted in 42.1% of cases. Statistical analysis showed that workaholism was not associated with socio-demographic characteristics. However, it was associated with smoking cigarettes, psychotropic drug consumption and poly-addiction and inversely associated with the presence of a leisure activity. With regard to occupational factors, workaholism was associated with agronomic engineering, working more than 8 hours per day, working the whole week and a job satisfaction score under 5/10.
Conclusion : Workaholism interested a significant proportion of this sample, and several professional factors could increase the likelihood of adopting this behaviour. The intervention of occupational doctors seems important in order to raise awareness about this form of addiction and to identify its early signs among employees. Methods : A descriptive and analytical cross-sectional study was carried out at the psychiatry department of Hassan II University Hospital in Fez over 12 months to compare three groups of patients: “schizo-obsessive” (n = 32), “schizophrenia” (n = 34), and “OCD” (n = 46). All participants (n = 112) were assessed using the mini-international neuropsychiatric interview (MINI), the Yale-Brown obsessive-compulsive scale (Y-BOCS), the Brown assessment of beliefs scale (BABS), the Hamilton anxiety rating scale (HAM-A), the Beck's depression inventory (BDI-II), the positive and negative syndrome scale (PANSS), and the clinical global impressions–severity scale (CGI-S). Results : The “schizo-obsessive” group differed from the “schizophrenia” group in: more severe psychotic symptoms (mean = 64.16 ± 17.049, P < 0.001), higher anxiety (mean = 8.87 ± 5,655, P < 0.001) and depression (mean = 7.50 ± 5.989, P < 0.001) scores, more prevalent suicide attempts (46.9%), higher illness severity score (mean = 5.13 ± 1.157, P = 0.02), and more professional disinsertion (78.1%). The “schizo-obsessive” group (mean= 14.47 ± 3.388) had significantly poor insight (P < 0.001) compared to the “OCD” group (mean= 8.35 ± 4.542). There were similarities in the obsessive and compulsive themes between the “schizo-obsessive” and the “OCD” groups, with no significant difference of severity (P = 0.26). Conclusion : A careful assessment of obsessive symptomatology is essential in schizophrenia for better patient management and prognosis. Our study presents preliminary data on clinical profiles of patients with anorexia nervosa (AN). We describe more specifically clinical characteristics of patients with early onset AN and according to their therapeutics orientations. Then, we compare the weight gain of patients managed only in day-patient (DP) treatment with those managed initially inpatient (IP) treatment and relays in DP. Methods : Ninety-two patients with AN, aged between 8 and 18 years, were evaluated with several questionnaires (EDI-2, EDE-Q, BSQ, EDS-R, CDI, STAI-Y, VSP-A, EPN-13). Results : Patients with early onset AN, n = 23 (25.3%), presented more restrictive behaviors, less marked dietary symptomatology, a lower degree of clinical perfectionism and a less marked feeling of ineffectiveness than adolescent patients with AN. Regarding the choice of hospitalization modality (DP alone or IP-DP), the only difference highlighted was the severity of patient undernutrition. Among the patients who were treated (IP-DP n = 27 vs DP alone n = 25), the weight evolution after one month and at discharge was favorable for both groups.
Conclusion : These preliminary data suggest the effectiveness of DP in the care of AN in children and adolescents. [résumé d'auteur]En ligne : https://www.em-premium.com/article/1632147 Permalink : https://ch-poitiers.fr/opac_css/index.php?lvl=notice_display&id=121075

This monocentric study aims to produce a French cross-cultural adaptation of a therapeutic alliance scale for the context of early consultations in a child and psychiatry department. Method : Fifty-five young children aged 3 to 30 months consulting for regulation or behavioral disorders and their mothers were included in the study. The working alliance inventory (WAI) was translated into French by two bilingual translators and adapted to early-age consultations in parent and therapist versions. Assessments of the child's symptoms and the parents’ anxiety and depression were carried out at the start and end of therapy. We studied the association of the alliance with the initial clinical characteristics and with the outcome of the child and the mother. An exploratory factor analysis was performed considering the items most associated with expected outcomes. Results : The alliance coded by the mother was lower in case of child behavioral problems and was associated with the mother and child outcome. Short versions of the infant-toddler WAI were developed based on factor analysis, highlighting four factors: positive goals and tasks, bond with the mother, alliance with the child, negative experience of care relationship. Discussion : Results were similar to those found in therapy with adults or older children. The alliance issue in mother-baby therapies was as essential as in other therapy contexts. The short Infant-Toddler WAI resulting from this work must be validated in future studies. Our objective was to describe the cumulative incidence and risk factors of anxiety disorders, including obsessive-compulsive disorder (OCD) and posttraumatic stress disorder (PTSD), in a follow-up of young adults over a five-year period. This is a prospective cohort conducted in two waves. The first took place from 2007 to 2009, in which 1,560 young adults aged between 18 and 24 years were evaluated using the Mini-International Neuropsychiatric Interview (MINI). Subjects were invited to participate in the second wave, which wave took place from 2012 to 2014, where 1,244 young adults were evaluated using the MINI-Plus. Our findings showed a cumulative incidence of 10.9% for any anxiety disorder, 6.5% for generalized anxiety disorder, 6.0% for agoraphobia, 2.0% for OCD, 1.6% for panic disorder, 1.1% for social anxiety and 0.7% for PTSD. Being female and having had a depressive episode were risk factors to develop any anxiety disorder. We observed a high cumulative incidence of anxiety disorders in a population-based sample of young adults. Our data highlights the importance of the early identification of these disorders as this could lead to early illness detection, early illness management and a reduced burden of disease. The prevalence of this phenomenon is not known in Tunisia due to the absence of validated assessment instruments. The aim of this study was to translate and validate the questionnaire 'Second Revision of the Revised Cyberbullying Inventory'. Methods : We translated this questionnaire into dialectal Tunisian Arabic using the back-translation method. To study the construct validity and the reliability, we conducted a cross-sectional study involving 962 Tunisian adolescents. Confirmatory factor analysis was performed to study construct validity for the two dimensions of the scale: cyber-victimization and cyber-aggression. To test reliability, the global internal consistency was computed for the two sections of the scale. Results : The translated version was considered satisfactory. The adjustment indices of the confirmatory factor analysis were satisfactory for both sections confirming the two-dimensional nature of the scale. The values of the cyber-aggression section were as follows: Comparative Fit Index = 0.92; Tucker-Lewis Index = 0.9; Root Mean Square Error of Approximation = 0.04; Standardized Root Mean Square Residual = 0.01. As for the cyber-victimization section, fit indices were as follows: Comparative Fit Index = 0.92; Tucker-Lewis Index = 0.9; Root Mean Square Error of Approximation = 0.01; Standardized Root Mean Square Residual = 0.07. Both sections showed good reliability. The internal consistency of each section was optimal. In fact, the Cronbach alpha was respectively 0.79 for cyber-aggression and 0.73 for cyber-victimization. Conclusion : The Arabic version of the 'Second Revision of the Revised Cyberbullying Inventory' is a psychometrically valid assessment. This scale could be useful to conduct further research and allow us to better understand the phenomenon of cyberbullying. Langues : Français (fre) Mots-clés : COVID-19 MULTIDIMENTIONAL SCALE OF PERCEIVED SOCIAL SUPPORT TUNISIE STIGMATISATION SOUFFRANCE PSYCHIQUE PANDEMIE PROFESSIONNEL DE SANTE Résumé : Background and objectives : Stigma was a major issue during the COVID-19 pandemic. It posed a serious threat to the lives of healthcare workers (HCWs) who were expected to experience higher levels of stigma and increased psychological distress. This is the first survey to investigate forms and correlates of perceived stigma in Tunisian HCWs during the COVID-19 pandemic. Methods : A cross-sectional web-based survey was conducted between October 8th and November 10th 2020, among 250 Tunisian HCWs. Data were collected using an online questionnaire using the Google Forms® platform. We used a self-reported instrument measuring COVID-19-related stigma, and the Multidimensional Scale of Perceived Social Support (MSPSS) to measure the perceived adequacy of social support from three sources: family, friends, and significant other. Results : The mean stigma score was 18.6 ± 8. Participants sometimes to often experienced stigma in their relationships with friends (22%), neighbors (27.2%), parents (22,4%), and in social activities (30.8%). This stigma was perceived mainly through avoidance (68.4%), and rarely through verbal (6%) or physical aggression (1.2%). The mean MSPSS total score was 5.26 ± 1.24. In multivariate analysis, depression history (P < 0.001), long working experience (P < 0.001), having presented ageusia/anosmia (P = 0.007) and lower total social support scale (P < 0.001) were significantly associated with higher perceived stigma score. Conclusion : Our findings showed that HCWs perceived stigma in professional, societal and familial domains. Social support from family, friends and others seemed to protect against perceived stigma. Proper health education targeting the public appears to be an effective method to prevent social harassment of both HCWs and COVID-19 survivors. Langues : Anglais (eng) Mots-clés : LECTURE DYSLEXIE PARENT ORTHOPHONIE REEDUCATION ENFANT ORTHOPHONISTE ETUDE COMPARATIVE Résumé : Background : Few studies have investigated the acceptability and efficacy of intensive parent-implemented interventions for children with dyslexia. Methods : We carried out a randomized controlled trial on 22 dyslexic children from 3rd to 5th grade. Reading performance was measured before (T1) and after (T2) summer by a selection of tests from the BALE and EDA batteries. One group received a specific parent-implemented repeated reading training (RR group) and the other group received a general training based on a summer vacation workbook (SVW group), adapted for children with reading impairment. The training lasted 6 weeks during the summer vacation. Results : In both groups, the reading performances of the dyslexic children were stable before and after the summer. No group difference was found on our primary outcome corresponding to an aggregate score of the z-scores of the BALE reading lists of regular, irregular and pseudo-words. However, secondary analyses revealed that the score of the EDA subtest “number of words read in one minute” (tapping reading fluency) differed significantly between the two groups (T2-T1 = 0.17 SD for the RR group and T2-T1 = -0.24 SD for the SVW group; P = 0.015). Acceptability was generally good (dropout rate of 9% in the RR group). Conclusions : A repeated reading intervention applied by parents may improve reading fluency of dyslexic children during summer vacation, with a good acceptability. Further studies are needed to confirm these findings. Disturbed identity appears as at the very core of this disorder, being inter-related with all other BPD features. Notably, from a dimensional perspective on mental disorders, one should realize that it is from our usual self-representation that we live all our daily experiences. Then, if the understanding of self-concept (or identity) is impaired, all the interventions implemented to decrease the self's suffering will subsequently be impaired. The purpose of the present case study was to illustrate the nine identity diffusion categories described by Jørgensen & Bøye (2022) and how the level of identity function can be improved in a third-wave cognitive and behavioral therapy targeting progressive correct self-identification. Langues : Anglais (eng) Mots-clés : RELATION FAMILIALE BOULIMIE ALIMENTATION IDENTITE RECHERCHE QUALITATIVE CULTURE D'ORIGINE ADOLESCENT ENFANT DE MIGRANT Résumé : Objectives : Bulimia nervosa (BN) is a common psychiatric disorder among adolescent girls with potentially significant complications. Family relationships play a major role in the development and progression of this disorder. Studies in migrant populations suffering from eating disorders show contrasting results depending on the generation of migrants: first generation migrants have fewer eating disorders than the native population, while the prevalence of this disorder is more important than the latter among second and third generation migrants. In our clinical experience, we have frequently encountered so-called “mixed” families, which are families composed of one migrant parent and one non-migrant parent. Research focusing on this kind of family is scarce which is why we chose to explore their dynamic. Methods : This study explored the issues around food and family relationships of adolescent girls suffering from BN, a topic that, to date, has not yet been studied. Ten interviews were conducted with five adolescent girls with BN between the ages of 16 and 20 and their parents, using photo-elicitation to enrich the collected data. Results : The results were organized around two axes: (1) identity issues around food, that is the assimilation process described by both parents and adolescents concerning family meals and food habits, and how the adolescents struggle to manage this interbreeding; and (2) transmission issues with the consequences the migrant parent has to deal with to transmit his/her cultural identity with food while being far away from the homeland, and the difficulties between this parent and his/her child to share this heritage. Both issues, identity and transmission, appear to be central among these families. Conclusions : Our results suggest a difficulty in mentalizing identity issues in adolescent girls; the function of appeasement around non-mentalized tensions was highlighted. In our opinion, in this particular context, BN acts as a means of expressing the difficulty of their mixed culture. This enables it to draw some clinical implications, especially using mentalization-based therapy which has already shown efficacy in adolescents with borderline personality disorder and ED. Mindfulness, defined as the awareness that arises by intentionally paying attention in the present moment and in a non-judgmental way, seems to be a moderator in the causal association between life stressors and well-being. Aims : The aim of the present study was to test the model of relationships between self-reported mindfulness, perceived stress, well-being index and life satisfaction in a sample of Tunisian HCPs. Methods : We performed a correlational cross-sectional study on a sample of HCPs at Hedi Chaker University Hospital, Tunisia. We used validated self-reporting instruments: the Mindful Attention Awareness Scale (MAAS), the Perceived Stress Scale (PSS), the World Health Organisation Well-Being Index (WHO-5) and the Satisfaction with Life Scale (SWLS). For data analysis, the SPSS version 23 program was used. Results : Participants (n = 297) included staff nurses (49.2%), medical technicians (19.2%) and doctors (31.6%). The overall model revealed that reduced perceived stress (β = −0.30, P < 0.000) and high levels of well-being (β = 0.13, P = 0.03) were associated with mindfulness, explaining 14% of the variance. Individually, perceived stress significantly predicted mindfulness explaining 13% of the variance. Conclusion : The study explores a variety of elements that can support the development of mindfulness interventions for stress prevention, staff well-being, and improvement of services as a whole. Langues : Anglais (eng) Mots-clés : FRANCE TROUBLE DEFICITAIRE DE L'ATTENTION HYPERACTIVITE PREVALENCE ADULTE ENFANT REVUE DE LA LITTERATURE Résumé : Objectives : Attention Deficit Hyperactivity Disorder (ADHD) is a frequent neurodevelopmental disorder whose first clinical description was published more than two centuries ago. Prevalence rates have been evaluated in many countries and meta-analyses conducted, but the data collected in France are scarce. Methods : A review of the literature was conducted by retrieving the references from four electronic databases leading to the selection of four studies in children and four in adults. Meta-analyses of prevalence rates were performed on this published data as well as unpublished results from the ChiP-ARD study (Children and Parents with ADHD and Related Disorders), separately for children and adults.
Results : While the quality of most studies is questionable, the prevalence rates are close to those reported in international meta-analytic studies for children (3.68%), but are higher in adults (5.5%). Conclusions : Well-conducted studies in both general and special populations are needed (e.g., in patients with depression, anxiety, bipolar disorder, developmental disorders including autism spectrum disorder, behavioural or substance abuse, and incarcerated). Nevertheless, healthcare stakeholders can conservatively consider that 0.8 million children and 1.4 million adults in the metropolitan French general population are likely to have ADHD and suffer from its multiple consequences.

Langues : Anglais (eng) Mots-clés : LAMOTRIGINE ETABLISSEMENT DE SANTE TROUBLE DE L'HUMEUR TROUBLE BIPOLAIRE PRESCRIPTION MEDICALE POSOLOGIE THYMOREGULATEUR PRATIQUE PROFESSIONNELLE PSYCHIATRE Résumé : Objective : Therapeutic drug monitoring for lamotrigine is poorly documented in bipolar and depressive disorders. In order to evaluate its use among French psychiatrists, we explored prescribing habits, therapeutic monitoring and dosage adjustment of lamotrigine through a flash survey. Methods : A survey was broadcasted by the network of Expert Centers for Bipolar Disorder and Resistant Depression and by the Collegial of Psychiatry of the Assistance publique des Hôpitaux de Paris. Questions concerned the frequency of prescribing depending on the mood disorder, the frequency of plasma levels, therapeutic monitoring, dosage adjustment and the limitation represented by dermatological risk. Results : Of the 99 hospital psychiatrists who responded, 66 practiced in a university hospital and 62 for more than 5 years. Overall, lamotrigine was more frequently prescribed for type 2 bipolar disorder (often: 51%) than for type 1 bipolar disorder (often: 22%). Dermatotoxicity was a major barrier to prescribing for 15% (n = 13) of respondents. Nearly two-thirds of prescribers (61%, n = 59) measured lamotrigine, of which 50% (n = 29) systematically. However, 40% of them did not have an opinion on the optimal plasma concentration. In total, 22% (n = 13) always adjusted the dosage according to the result. The first argument for dosage adjustment was clinical response for 80% (n = 47) of prescribers, adverse effects for 17% (n = 10) and plasma levels for only 4% (n = 2). Conclusion : While many psychiatrists report using plasma dosage of lamotrigine, few use the plasma level result to adapt dosage and many have no opinion of the target values for plasma concentrations. This illustrates the lack of data and recommendations regarding the use of therapeutic pharmacological monitoring of lamotrigine in bipolar and depressive disorders. Langues : Anglais (eng) Mots-clés : FRANCE PSYCHIATRIE MEDICO LEGALE UNITE POUR MALADES DIFFICILES HOSPITALISATION PSYCHIATRIQUE EPIDEMIOLOGIE Résumé : Introduction : Basic epidemiological data are rare concerning the activity of specialized forensic psychiatric facilities in France. Here, we investigated the activity of the ten (640 beds) French “units for difficult patients” (unités pour malades difficiles [UMDs]). Method : We used the Programme de médicalisation des systèmes d’information (PMSI) database to describe the characteristics and evolution of psychiatric hospitalisations in UMDs between 2012 and 2021, as well as the age, sex, and principal diagnoses of the patients hospitalized in these facilities. Results : Between 2012 and 2021, 4857 patients were hospitalized in UMDs (6082 stays). Among them, 897 (18.5%) had more than one stay. The number of admissions ranged from a minimum of 434 to a maximum of 632 per year. The number of discharges ranged from a minimum of 473 to a maximum of 609 per year. The mean length of stay was 13.5 (SD: 22.64) months with a median of 7.3 months (IQR: 4.0–14.4). Among the 6082 stays, 5721 (94.1%) involved male patients. The median age was 33 (IQR: 26–41) years. The most frequent principal psychiatric diagnoses were psychotic disorders and personality disorders. Conclusion : The number of individuals hospitalized in specialized forensic psychiatric facilities has been stable for 10 years in France and remains lower than in most European countries.
